Naming: YYYY-MM-DD-slug.md per issue/brief (e.g. 2026-09-27-waf-strict-boundary.md).

Shape:

• Finding / issue — what, severity, source.
• Remediation — the fix, with evidence it was applied.
• Gate state — which gates pass, which are held, by whom.

Owner rulings and exceptions are recorded verbatim, not summarized.
